Yes, the last apple refers to ones cheekbones, the so-called “apples of youth”.

Over the past few years, everyone surely has heard of the expression “apples of youth” or even “apple muscles”. It doesn’t refer to an anatomy part (these muscles don’t actually exist), but it’s a rather popular concept in aesthetic medicine to indicate fat accumulated in the right places.

The “apples of youth” are in fact subcutaneous tissues and fatty fascia located on the cheekbones, about 2cm below the eyes, in the shape of an inverted triangle; when smiling, they protrude slightly due to the tension of facial muscles, thus resembling a pair of round shiny apples.

Many people can't tell the difference between “apple muscles” and high cheekbones because their positions are similar, but they’re quite easy to tell apart. Cheekbones are, as the name suggests, a bone, and will be visible regardless of whether a person is smiling or not, whereas the “apples of youth” will only become apparent with a smile.

Another difference between the two is linked to age: as time passes and the skin starts to lose its tension, the position of the “apple muscles” will slowly change, they will start shrinking and lowering because of gravity, while the cheekbones will stay in their place.

Sometimes, people even stop having “apples of youth” after losing some weight.

When the “apples” start losing their shape, they can pull a lot of skin with them, causing a drop in the eyelids, a deepening of the tear troughs, and the formation of many nasolabial folds called “wrinkles of sadness”, making a person look 8-10 years older. Keeping the “apples” high and tight is what helps maintaining a pretty and youthful visage.
